Overview

Direct liaison between the patient, physicians and other departmental staff. This position is also responsible for overseeing the drug screening program. Provides a minimal amount of direct patient care.

Responsibilities

  • Oversees the Prescription refill line and assists in processing prescription refills and edit prescriptions as deemed necessary.
  • Responsible for reviewing the opioid treatment agreement with all new and existing patients
  • Provides basic patient care as ordered.
  • Responds to life threatening situations based upon nursing standards, polices, procedures and protocols.
  • Being courteous, providing excellent customer service to patients, visitors, providers and co-workers.
  • Answering incoming telephone calls and refers calls as necessary. Also responsible for taking messages, paging doctors when appropriate, handling hospital consults, and answering inquiries.
  • Notifying patients when their prescriptions are ready for pick-up for all providers
  • Distributing prescriptions according to office protocol
  • Ensuring that appropriate phone calls are transferred
  • Scanning all correspondence and medical reports daily as it relates to patients.
  • Appropriately document all nursing interventions in the electronic medical record. Review the chart for completeness, test results and billing.
  • Scans upcoming appointments for additional medical record information necessary. Obtain medical records from other physician offices, hospitals, labs, emergency rooms, and radiology departments prior to patient visit.
  • Completes all “tasks” assigned to individual team (as well as covering for other nurses/MA’s who may be temporarily absent), including responding to patient messages, prescription refills, lab results, referrals, home health agencies, medication prior authorizations, patient forms and follow up from physicians. All tasks must be reviewed, addressed and marked appropriately before the close of each business day.
  • Responds to questions from patients, family members, faculty/residents, other health care providers in a timely and professional manner. Patient phone calls should be answered on the same day they are logged in, before leaving for the day. If patient is not home at the first attempt, a notation should be made in the medical record.  All patient conversations must be documented in the electronic medical record.
  • Follows EVMS protocols related to infection control, environmental safety and patient confidentiality. Complete annual recertification for OSHA, Blood Borne Pathogens, Compliance, Respiratory FIT and IT policy. Maintain current CPR certification and nursing license.
  • Nursing staff is required to fill in for other clinical staff during vacations and illness and perform other duties as assigned.

 

Oversee Drug Screening Program including monitoring of patient results and arranging appropriate follow-up and response and  maintaining  good knowledge base regarding federal regulations

 

  • Reviewing opioid agreement with all new and existing patients.
  • Oversees the Prescription Monitoring Program; to include reviewing prescription monitoring reports.
  • Overseeing/Assisting with Urine Drug Screenings:  collection process & assuring chain of custody
  • Consults laboratory toxicologist periodically for insight/questions
  • Initiates random/routine pill count/drug screening per protocol
  • Orders and review urine drug screens per protocol/oral fluid tests/serum blood reports
  • Responds to patient calls regarding clinical care questions/concerns and facilitating physician reports
  • Monitors/reviews physician tasks in the EHR and assists as appropriate.
  • Print and facilitates prescription refills as protocol
  • Carries out discharge/reinstatement orders from physicians as protocol
  • Provides patient and family education, create hand-outs
  • Reviews opioid agreements with new and existing patients and educates patients that may be in violation of the opioid agreement/office policy.
  • Consults with Physicians, Chair/Medical Director or Practice Manager on nursing problems and interpretation of departmental policies to ensure patient needs are met
  • Assists with prescription request and/or obtaining authorization for specific medications or procedures as needed

Oversees care management support with patients that are at risk of discharge by developing a mutually agreed upon written care plan that may include more frequent interventions and counseling.

 

  • Assists providers as it relates to patient education and care management and provides patient education specific to that patient’s situation
  • Monitors and complete tasks lists
  • Performs other clinical duties as assigned

 

Qualifications

Associates degree plus two years of relevant experience working in a hospital or physician's office. Must have working knowledge of computers. Customer service oriented individual is required. Licensed Practicing Nurse is preferred (must possess valid VA. License).
