  • Overview

    Provides comprehensive HIV/AIDS case management, patient education and prevention services of some difficulty to HIV(+)/AIDS patients, including individualized case management and counseling.  Assists clients with the eligibity and enrollment procedures for various federal/state programs. Collect and maintain accurate program data. Makes appropriate psycho-social and clinical referrals. Performs patient education in HIV/AIDS prevention and medical treatment adherence.

    Responsibilities

    • Uses variety of skills to evaluate and assist patients of the Center for Comprehensive Care of Immune Deficiency (C3ID) in overcoming barriers to proper treatment and management of their HIV disease.
    • Is familiar with the current Virginia Department of Health HIV/AIDS Case Management Guidelines and provides at least minimum standard of care consistent with those guidelines.
    • Provides confidential screening of referred patients by telephone or face-to-face encounters for eligibility under various entitlements programs. 
    • Assesses patient’s individual strengths and weaknesses on psychosocial issues. 
    • Develops individualized, need specific service plans to assist the patient with barriers to medical care and general wellness including, access to medications, housing and other financial needs, dental services, transportation, mental health and/or substance abuse issues. 
    • Provides periodic follow-up face-to-face and by telephone; although rare, occasional home visits may be required. 
    • Provides necessary linkages to government entitlements programs including Social Security Disability determinations, Medicaid, Medicare, Ryan White, and AIDS Drug Assistance Programs (ADAP) that may include assistance in completing application forms and occasionally accompanying patient to respective appointments. 
    • Collect, review and maintain appropriate financial and supporting documentation to support patient eligibility on periodic basis.
    • Developed and maintains strong linkages to community Aids Service Organizations (ASO’s). 
    • Maintains expertise in substance abuse & mental health issues; provides counseling as needed in those areas. 
    • Provides crisis intervention to HIV/AIDS patients per primary care provider referral. 
    • Provides reinforcement counseling on adherence to anti-retroviral therapy to patients on a routine basis and refers to the Adherence Counselor when there is evidence of non-compliance. 
    • Accesses indigent patient pharmaceutical company drug programs to supply medications to eligible patients. 
    • Actively participates in case conferences as necessary with patient’s physician, mental health counselor, and medical counselor to develop a comprehensive plan for management of patient’s HIV disease. 
    • Stays abreast of new therapies in management of HIV disease and changes in HIV health care delivery systems. 
    • Orients patients under case management to services available at C3ID. 
    • Maintains an up-to-date list of all Ryan White eligible patients and provides monthly to the Case Manager Supervisor and others as necessary. 
    • Provides and explains to client Patient Bill of Rights and Responsibilities, Grievance Procedures, Advance Directives and Power of Attorney and other patient information as necessary. 
    • Documents in a timely manner in chart form all of the patient encounters including the assessment, specific action expected of patient, necessary action of case manager, date of follow-up, etc. 
    • Regularly attends required modules, conferences and meetings during and after normal work hours to maintain high level of competence in relevant fields.
    • Enter into CAREware system all patient demographics, monthly encounters, case notes and other required data.  Prepare grantee reports as necessary. 
    • Performs other duties as assigned.

    Qualifications

    EDUCATION:

    Bachelor's degree in a human services-related field, such as social work, psychology, nursing, counseling, health sciences or education or Registered Nurse.

     

    EXPERIENCE:

    1 year of experience, preferably in a human services-related field such as social work, psychology, nursing, counseling, health sciences or education or Registered Nurse.
