Overview

The Advanced Practice Provider (APP) will play a pivotal role within the EVMS Department of Medicine, Hospitalist Division, with a focus primarily on night shifts. In this dynamic position, APP will take charge of managing health issues and coordinating healthcare in progressive care or acute care in-patient hospital settings. As an integral part of our team, the APP holds the responsibility of delivering top-notch patient care and medical services to individuals, families, and/or groups within the hospital setting. This includes working independently and collaboratively with a sponsoring physician, and internal medicine residency program teaching teams, providing a wide range of health care services. The scope of practice encompasses conducting thorough health assessments, diagnosing medical conditions, developing personalized medical plans of care, and prescribing/providing treatments and interventions. APP will also play a key role in evaluating patient responses, making referrals, and adeptly managing both acute and stable chronic illnesses. This position offers a unique opportunity to work independently during overnight shifts, showcasing autonomy and expertise. APPs will have access to remote physician support as needed, ensuring a collaborative and supportive environment.

 

Will include a night shift differential in pay. 

Responsibilities

  • Able to evaluate medical problems of patients in the in-patient setting, devise a management plan of care including further testing and medications, and appropriately requests consultation of other specialties when indicated.
  • Provides education to patients and/or families about preventive care, medical issues and use of prescribed medical treatments and/or medications pertinent to the patient’s condition.
  • Creates diagnostically appropriate treatment plans
  • Documents all medical evaluation, diagnoses, procedures, treatments, outcomes, education, referrals and consultations consistent with NCQA, The Joint Commission, state regulatory standards and evidenced-based standards of care.  Maintains documentation compliance for appropriate coding and billing.
  • May refer complex and high priority cases to collaborating physician with regards to complicated diagnostic problems, serious illness, complicated therapeutic problems and re-evaluation of chronic conditions.
  • Provides medical and emergency interventions appropriate to the patient’s needs; prescribes and monitors medications appropriate to the diagnosis; orders and monitors the appropriateness of emergency interventions.
  • Maintains collegiality with all members of the inter-disciplinary team.
  • Utilizes appropriate chain-of-command and communicates with Division Chief, or designee, any clinical issues appropriately and contributes to quality improvement or process improvement as necessary.
  • Must be able to perform the professional, clinical and or technical competencies of the hospitalist role.
  • Initial assessment of the patient’s service needs;
  • Develops treatment plans for patients and or revises in accordance with applicable protocols and standards of care.
  • Differentiates between normal and abnormal findings and orders appropriate diagnostic studies.
  • Counsels’ patients and their families, when necessary, in areas such as health maintenance,
  • medication adjustments and end of life issues.
  • Maintains strict confidentiality of patient information in accordance with HIPAA and handles all
  • matters professionally with sensitivity to racial, ethnic, and sexual orientation issues.
  • Maintains effective working relationships with faculty, staff, residents and the public.
  • Participate in development and implementation of divisional and/or departmental quality assessment/improvement/research programs and processes as requested.
  • Contributes toward the achievement of divisional and/or departmental goals.
  • Participates and contributes in/toward activities that enhance clinical expertise.
  • Participates in professional clinical activities in the Department, Division, community and professional organizations as needed or requested.
  • Signs out patients at the end of clinical work
  • Performs other duties as assigned.

Qualifications

  • Nurse Practitioners must have an MSN with a nurse practitioner focus.  Must be a licensed RN by the Virginia Board of Nursing and a certified nurse practitioner. 
  • Physician Assistants must have an MPA. Must be a licensed Physician Assistant within the Commonwealth of Virginia.
  • BLS and ACLS Certification required.
  • Age-appropriate training for “population served” may depend on NP or PA program training and may require additional training as determined by the Hospital Medicine Division.
