Overview

Assesses, instructs, educates and follows patients with diabetes as established per standards of care and provides nursing services.  Serves as the Program Coordinator for the American Diabetes Association (ADA) related to adherence to national standards for Diabetes Self- Management Education.

Responsibilities

Curriculum Development, Academic Skills and Teaching

 

  • Design a diabetes education curriculum to include all content areas defined by the ADA.

 

  • Individualize the curriculum to meet the immediate needs of patients with barriers to completing the entire educational curriculum experience.

 

  • Prioritize patient needs and assess patients learning ability.

 

  • Interact with patients one-on-one and in structured group diabetes classes to provide for optimal diabetes self-management strategies and skills for the patient/families.

 

  • Evaluate patient progress, outcomes, and goal achievement

 

  • Speak effectively before groups of patients, healthcare providers, and the general public regarding standards of medical care in diabetes and diabetes self-management education.

 

  • Interpret and analyze documents, professional journals, clinical protocols, and procedure manuals

 

Management and Organization

 

  • Oversee and implement the planning, implementation, and evaluation of the diabetes self-management program which includes the following:

 

  • Curriculum development/update according to national standards of care
  • Implementation of CQI projects for quality improvement
  • Completion of ADA applications, and annual status reports to maintain ADA Recognition

 

  • Collaborate with multi-disciplinary diabetes health care specialists to implement quality education programs, and assist with recommending treatment plans.

 

  • Monitor and analyze data to improve patient outcomes, and program objectives.

 

 

Patient Care

 

  • Assesses individual patient needs and develop tailored educational programs for patients with diabetes.

 

  • Evaluate the impact and effectiveness of these programs.

 

 

  • Prepares plans for family support and participation.

 

  • Assists in the development, and revision of educational tools, and media used in the clinical setting.

 

  • Documents in the patient’s medical record any education provided.

 

  • Provides nursing support: vital signs, blood glucose testing, EKG’s, retinal photos, etc.

 

  • When appropriate, make referrals for additional services.

 

  • Performs other duties as assigned

 

Qualifications

  • BSN is required.
  • Must be licensed RN by the Virginia Board of Nursing.
  • Certified Diabetes Educator (CDE) or Board Certified in Advanced Diabetes Management.

 

EXPERIENCE

 

  • Two (2) years of experience is required.
